Please complete this form to request a cancellation or pause of your athlete’s membership. As outlined in the membership agreement signed at enrollment, True Athlete Performance requires 30 days written notice for all cancellations or pause requests.

Please note

Memberships remain active during the 30-day notice period.
Scheduled payments during the notice period will still be processed.
Cancellation requests cannot be backdated.
You will receive confirmation from a member of our team within 1–2 business days after submitting this request.
